Technique

Why the technique decides the outcome

Lipedema tissue is fibrous and rich in lymphatics. The instrument and the direction of work determine both safety and contour.

WAL

Water-jet assisted liposuction

A fine, fanned water jet loosens fat cells from the surrounding structures before they are aspirated. Lymph vessels, nerves and connective tissue are largely spared.

  • Gentle on lymphatics – the standard in lipedema
  • Less swelling and bruising than dry techniques
  • Feasible in tumescent local anaesthesia
VAL

Vibration assisted liposuction

A vibrating cannula works efficiently through fibrous, nodular tissue – typical of advanced stages and of areas treated before.

  • Effective in fibrotic tissue
  • Even contour in stage II and III
  • Frequently combined with WAL in one session
Planning

Region-based sessions

We treat in anatomical units so that contours stay balanced: outer and inner thighs, knees, lower legs, arms.

  • Usually two to four sessions
  • Six to twelve weeks between sessions
  • Volume limits per session for safety
Aftercare

Compression and lymphatic support

Compression for six weeks after each session, lymphatic drainage where useful, early mobilisation from day one.

Anaesthesia

Tumescent local or general anaesthesia

Both are possible. The choice follows the volume, the number of regions and your own preference.

From planning to final contour

    Planning appointment

    Confirmation of diagnosis and stage, definition of regions and sessions, measurement, photographs and the written GOÄ cost estimate.

  2. 02

    Preparation

    Anaesthetic review, laboratory tests, no nicotine, review of anticoagulants, compression garment ordered and fitted in advance.

  3. 03

    Session

    Markings while standing, tumescent infiltration, lymph-sparing aspiration using WAL and VAL, small access incisions left open for drainage.

  4. 04

    First two weeks

    Compression day and night, walking from the first day, wound checks, lymphatic drainage where indicated. Swelling and bruising are expected.

  5. 05

    Result and next session

    The contour settles over three to six months. The next region follows after six to twelve weeks.

Preparation

What we ask of you

  • No nicotine for at least two weeks before and after each session
  • Anticoagulants only paused after agreement with your treating doctor
  • Compression garment ordered and fitted before the date
  • Someone to accompany you home on the day of surgery
  • Plan the healing phase: walking yes, sport no, for six weeks

With water-jet assisted technique and cannulas guided along the longitudinal axis, lymphatic injury is very rare. That is precisely why we use WAL and VAL rather than dry aggressive techniques.
