Lipedema: diagnosis, stages and surgical treatment

Lipedema is not a question of willpower. It is a chronic fat distribution disorder – painful, progressive and resistant to diet. We treat it surgically with lymph-sparing liposuction using WAL and VAL.

Private practice: German statutory funds can cover liposuction from stage I, but we do not bill statutory funds. Treatment with us is private or self-pay.

Step 1

Diagnosis and staging

Lipedema is a clinical diagnosis: symmetrical fat distribution disorder of the limbs with pain on pressure and touch, a tendency to bruise and a disproportion that diet does not resolve.

Step 2

Conservative measures

Compression, manual lymphatic drainage and exercise reduce symptoms but do not remove diseased fat tissue. We treat them as a supportive framework, not as our main service.

Step 3

Surgical treatment

Lymph-sparing liposuction using water-jet assisted (WAL) and vibration assisted (VAL) technique removes the pathological fat tissue permanently.

  • Usually two to four sessions
  • Tumescent local anaesthesia or general anaesthesia
  • Compression for six weeks per session
Recognising lipedema

Typical findings

If several of these points apply to you, a structured assessment is worthwhile.

  • Symmetrical increase in volume of legs and/or arms, with the feet and hands spared
  • Pain on pressure, tension and a feeling of heaviness, worse in the evening and in heat
  • Bruising after minimal trauma
  • Disproportion between trunk and limbs that persists despite weight loss
  • Little or no response to diet and training in the affected areas
  • Frequently a family history in mothers, sisters or daughters

Frequently asked questions

Since the 2024 decision, statutory funds in Germany can cover liposuction for lipedema from stage I onwards under defined conditions. However, we do not work with statutory funds: the techniques we use – in particular VAL and water-jet assisted liposuction with the corresponding theatre and anaesthesia standard – cannot be delivered at statutory reimbursement rates. Treatment with us is private or self-pay.
